Bridge Program
The Bridge Program provides an alternative to nursing home care for those older Pennsylvanians who are not yet eligible for the PDA Medicaid Waiver Program.
As with the Medicaid Waiver program, individuals found to be clinically appropriate and eligible for nursing home placement are served in their own homes and provided more intensive community and in-home services.
A care plan is developed
to insure adequate care and safety, and intensive monitoring is provided to
maintain the consumer.
